What to Expect

Your first visit starts with a consultation. We'll review your health history, take any needed imaging, and walk through your treatment plan and options together in plain language.

If you complete your registration form ahead of time, your visit can start with your consultation instead of paperwork. Please arrive 15 minutes early for your first visit.

Before Your Appointment

Getting Ready for Your Visit

Save time at your first visit by completing your registration form online beforehand. It covers your contact information, health history, and insurance details.

Complete Registration Form

Please also bring the following with you:

Your referral to the oral surgeon
A list of medications you are presently taking
Your dental and medical insurance cards
A parent or guardian, if the patient is under 18
X-rays, if available — or have your dentist email them to info@vernonhillsoralsurgery.com

Insurance & Payment Options

We stay in-network with most medical and dental insurance plans, so cost isn't the reason you delay care.

For costs not covered by insurance, we offer flexible payment options. Ask our front desk team for details specific to your treatment plan.

Please remember that you are fully responsible for all fees charged by this office, regardless of your insurance coverage.

Our team is happy to help file your dental or medical claims, and we simply ask that payment follow our standard financial policy. Feel free to ask our staff for details at your consultation.
